The facility is in satisfactory compliance with state law. No major violations found.

B

Needs Improvement

Score 80–89

Some violations were found and must be corrected. A B grade often triggers a follow-up re-inspection within 14–30 days.

C

Failing Grade

Score 79 or below

Serious violations. The facility may face a closure order until corrections are made and confirmed by re-inspection.
